ABSTRACT

PURPOSE: Breast cancer is the most commonly occurring cancer among women worldwide, and therefore, improved approaches for its early detection are urgently needed. As microRNAs (miRNAs) are increasingly recognized as critical regulators in tumorigenesis and possess excellent stability in plasma, this study focused on using miRNAs to develop a method for identifying noninvasive biomarkers. METHODS: To discover critical candidates, differential expression analysis was performed on tissue-originated miRNA profiles of 409 early breast cancer patients and 87 healthy controls from The Cancer Genome Atlas database. We selected candidates from the differentially expressed miRNAs and then evaluated every possible molecular signature formed by the candidates. The best signature was validated in independent serum samples from 113 early breast cancer patients and 47 healthy controls using reverse transcription quantitative real-time polymerase chain reaction. RESULTS: The miRNA candidates in our method were revealed to be associated with breast cancer according to previous studies and showed potential as useful biomarkers. When validated in independent serum samples, the area under curve of the final miRNA signature (miR-21-3p, miR-21-5p, and miR-99a-5p) was 0.895. Diagnostic sensitivity and specificity were 97.9% and 73.5%, respectively. CONCLUSION: The present study established a novel and effective method to identify biomarkers for early breast cancer. And the method, is also suitable for other cancer types. Furthermore, a combination of three miRNAs was identified as a prospective biomarker for breast cancer early detection.

This paper detailed the Coleman concept and practice of coach-led transitional care of elderlies,team members and their responsibilities,intervention aspects and main tasks,as well as the intervention program and its effect analysis.Then the authors analyzed the present transitional care development in China,and recommended on development of a likewise care model for senior chronic disease patients in the country,such as building multi-disciplinary transitional care teams to provide uninterrupted care and service,encourage patients and caregivers to embrace self-management of diseases,and build an appraisal mechanism to ensure quality of care.

ABSTRACT

Voriconazole is a broad spectrum triazole antifungal agent, widely used in the prevention and treatment of invasive fungal diseases.Long-term use of voriconazole can induce periostitis and cutaneous squamous cell carcinoma, accounting for serious adverse effects on patients, which has been reported in increasing clinical cases.This review is to characterize the epidemiological and clinical manifestations of periostitis and squamous cell carcinoma induced by voriconazole, and to analyze the mechanisms of triggering periostitis and squamous cell carcinoma, so as to promote the rational use of voriconazole in clinics.

ABSTRACT

Objective@#To study the clinicopathologic features of plasma cell myeloma(PCM) with bone marrow fibrosis (MF).@*Methods@#The clinicopathologic data of 175 cases of newly diagnosed PCM patients were retrospectively analyzed. Based on reticular fiber staining, these cases were divided into PCM-MF and non-PCM-MF groups.@*Results@#Sixty-three cases were PCM-MF(36%), 112 were non-PCM-MF (64%). No statistical difference in gender, age, hemoglobin level, platelet counts, the classification of immunoglobulin, ISS staging, immunohistochemical phenotypes and genetic features was found between PCM-MF and non-PCM-MF groups (P>0.05). Compared to non-PCM-MF group, lactate dehydrogenase (LDH)level and renal impairmentrate were higher in PCM-MF group (P<0.05). The degree of bone marrow hyperplasia, the percentage of myeloma cells and cells with plasmablastic morphology were significantly higher in PCM-MF group(P<0.05).@*Conclusion@#The higher LDH level, renal impairment rate, and more significant bone marrow hyperplasia, proliferation of plasma cells and plasmablastic myeloma cells infiltration indicate poor prognosis of PCM-MF patients.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the impacts on post-stroke unilateral spatial neglect treated with acupuncture for "regaining consciousness, benefiting marrow and opening orifices" and rehabilitation.</p><p><b>METHODS</b>Forty patients of post-stroke unilateral spatial neglect were randomized into an observation group and a control group, 20 cases in each one. In the observation group, acupuncture was applied to the acupoints for "regaining consciousness, benefiting marrow and opening orifices", named Baihui (GV 20), Sishencong (EX-HN 1), Benshen (GB 13), Shenting (GV 24), and the bilateral-primary points and-connecting points of the heart meridian, pericardium meridian and kidney meridian, as well as the acupoints along the affected meridians. Additionally, the rehabilitation was provided. In the control group, acupuncture at the acupoints along the affected meridians and rehabilitation were adopted. The treatment was given once a day, 5 times a week. After 8 weeks of treatment the evaluation was made. the indexes of unilateral spatial neglect such as line bisection test, the score of the mini-mental state examination (MMSE), the score of simple Fugl-Meyer motor function assessment and the modified Barthel indexes were adopted for the assessment of the severity of unilateral spatial neglect, cognitive function, motor function, and the activities of daily living.</p><p><b>RESULTS</b>After treatment, the indexes of unilateral spatial neglect (line bisection test, line cancellation test, clock-drawing test and copying drawing test), MMSE score, the simple Fugl-Meyer motor function assessment and modified Barthel indexes were all improved as compared with those before treatment in the two groups (all<0.01). The improvements in the observation group were more obvious than those in the control group (<0.05,<0.01).</p><p><b>CONCLUSION</b>The combination of acupuncture treatment for "regaining consciousness, benefiting marrow and opening orifices" and rehabilitation much more effectively alleviates the severity of post-stroke unilateral spatial neglect and improves the motor function and the activities of daily living in the patients.</p>

ABSTRACT

OBJECTIVE:To provide reference for perioperative rational prophylactic application of antibacterials in type Ⅰ in-cision operation. METHODS:4 201 patients underwent type Ⅰ incision operation were collected from a class A grade three hospi-tal during Mar. 2013-Feb. 2015. The perioperative prophylactic application of antibacterials in type Ⅰ incision operation were ana-lyzed statistically. RESULTS:Of 4 201 patients underwent type Ⅰ incision operation,there were 2 399 cases of prophylactic appli-cation of antibiotics(accounting for 57.10%). Cephalosporins andβ-lactam/β-lactamase inhibitor were the main classes of antibacte-rials for preventive use,accounting for 45.60%and 19.76%,respectively. The frequency of using cefazolin,ceftriaxone and amoxi-cillin-clavulanic acid ranked the top 3 places, among which there was 823 cases of unsuitable prophylactic medication time (34.31%),and 855 cases of prophylactic medication time >48 h(accounting for 35.64%). CONCLUSIONS:There still is unrea-sonable perioperative prophylactic application of antibacterials in type Ⅰ incision operation in this hospital. It is necessary to strengthen the supervision of antibacterials and conduct regular rational antibacterials use seminars in order to promote rational use of antibacterials.

ABSTRACT

Objective To evaluate whether S-100β protein could be a serum marker for traumatic spinal cord injury (SCI). Methods From June, 2013 to October, 2014, 24 patients with complete SCI were measured the serum S-100β protein concentrations with en-zyme-linked immunosorbent assay, one week, three and six months after SCI. Serum from ten healthy persons was as normal control. Re-sults The serum S-100βprotein concentrations increased one week and 3 months after SCI (Z>4.273, P<0.001). Conclusion The increase of serum S-100βprotein may help assessing early impairment after complete SCI.

ABSTRACT

@#Objective To summarize experience of nursing for thoracic breathing reconstruction in patients after cervical spinal cord in-jury (CSCI). Methods 6 cases accepting thoracic breathing reconstruction in our hospital from March 2013 to June 2014 were reviewed from 1 week before operation to 4 weeks after operation. Results All the patients, aged (41.7 ± 16.2) years, received tracheotomy (6.0 ± 5.8) days after injury because of weakened cough, retention of respiratory secretions and dyspnea;the ventilator was used for (45.3±25.9) days, and they received thoracic breathing reconstruction surgery (84±26.7) days after injury. Their vital capacity was (1085±92) ml before opera-tion and was (1680±283) ml 2 weeks after operation with 55%increment (P<0.01). Only 1 patient underwent a delayed wound healing. No pulmonary infection, urinary tract infections and pressure sore occurred until 4 weeks after operation. Conclusion Enhancement of airway care and prevention of pressure sore are the keys to reduce postoperative complications of thoracic breathing reconstruction.

Objective To probe the therapeutic effect of ginger-salt-partitioned moxibustion on urge incontinence after stroke. Methods 40 stroke patients following urge incontinence were randomly divided into control group (n=20) and experimental group (n=20). 2 groups all received the same treatment, routine acupuncture and rehabilitation. Additionally, the experimental group received ginger-salt-partitioned moxibustion at Shenque (CV8). The curative effect was compared after treatment (4 weeks). Results 36 cases finished the treatment, 17 in the control group and 19 in the experimental group. Total number of urination, urinary incontinence and nocturia reduced, and the average volume of each urine increased (P0.05). Conclusion The effect of ginger-salt-partitioned moxibustion at Shenque on poststroke following urination disorders is remarkable.

@#Objective To investigate the effects of electroacupuncture on head Shen- acupoints on cognitive dysfunction after stroke. Methods 35 cases with cognitive dysfunction after stroke were divided into treatment group (n=19) and control group (n=16). The control group received conventional treatment, while the treatment group received electroacupuncture on Sishencong (EX-HN1), Shenting (GV24) and Benshen (GB13) in addition. They were assessed with Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA) before and after 4-week treatment. Results The scores of MMSE and MoCA increased more in the treatment group than in the control group (P<0.05). The scores of visuospatial abilities and executive functions, attention and concentration, calculation, memory and orientation of MoCA improved more in the treatment group than in the control group (P<0.05). Conclusion Electroacupuncture on head Shen-acupoints can improve cognitive function in patients after stroke.

ABSTRACT

Objective To investigate the association of Galectin-3 ( lectin, galactoside-binding soluble , LGALS3)gene-191(rs4644)polymorphism and breast cancer in Chinese female population .Methods Galectin-3 gene rs4644 polymorphisms was genotyped by polymerase chain reaction-restriction fragment length polymor-phism(PCR-RFLP)in 172 breast cancer patients and 17 controls.The experimental results of PCR-RFLP were verified and sample of 10 cases were sequenced randomly .Results The frequency of CC ,CA and AA genotype in breast cancer group and normal control group was 72.7%, 25.0%, 2.3% and 52.9%, 41.2%, 5.9%, respectively .The frequency of C and A allele in breast cancer group and normal control group was 85.2%, 14.8%and 73.5%, 26.5% respectively .The genotype and frequency of allele had no statistical difference between breast cancer patients and the control group .rs4644C allele was more likely to occur in breast cancer patients with T>2 cm (χ2 =8.295,P=0.004) and Ki67<14%(χ2 =3.917,P=0.048).Conclusions There is no significant association between Galectin-3 gene 191 polymorphism rs4644 and susceptibility of breast cancer in Chinese female population .It is closely associated with the size of tumor and Ki 67.

@#Objective To study the effects of body weight support treadmill training (BWSTT) or Du channel electroacupuncture (DcEA) on locomotion function and expressions of nerve growth factor (NGF) and growth-associated protein-43 (GAP-43) in rats with spinal cord transection. Methods 54 adult SD rats were divided into control group (n=18), BWSTT group (n=18) and DcEA group (n=18). Each group was divided into 3 subgroups (n=6) for 8 d, 15 d and 30 d respectively. Spinal cord transection was performed at T10 level. The locomotion function was assessed with Bosso-Beattie-Bresnahan locomotor rating scale for open field (BBB scale) and the expressions of NGF and GAP-43 in spinal cord tissues to the injury caudal ends were measured with immunohistochemistry. Results The scores of BBB scales and expressions of NGF and GAP-43 were higher in BWSTT group and DcEA group than those in the control group on the 8th d,15th d and 30th d respectively (P<0.01); and were higher in DcEA group than in BWSTT group on the 15th d and 30th d respectively (P<0.01). Conclusion Both BWSTT and DcEA can improve the locomotion function in rats with spinal cord transection and increase expressions of NGF and GAP-43 in spinal cord tissues, and DcEA is more effective.

@#The spasticity can affect the motor function, activities of daily living (ADL), quality of life (QOL) of hemiplegic patients,and also delay the rehabilitation process. Principally, the methods of reducing spasticity included physical therapy and medication at present.The massage technique has been having outstanding effect on alleviating spasticity after stroke. This article reviewed the clinical efficacy,methods and mechanisms of massage treatment for hemiplegic spasticity.

ABSTRACT

Objective To study the effect of swallowing training combined with acupuncture on patients with dysphagia more than one month after stroke or traumatic brain injury (TBI). Methods 61 patients with dysphagia meeting the inclusion criteria were randomly divided into a control group (31 cases) and a treatment group (30 cases). Both groups were subdivided according to the course of disease-1 ~ 2 months or > 2 months. The patients in the control group received only swallowing training, 30 min per day, 5 times a week for 9 weeks. The patients in the treatment group were treated with the same swallowing training as the control group, additionally combined with acupuncture at the acupoints around the neck, including Lianquan (Ren23), Jinjin (EX-HN12), Yuye (EX-HNI3),Fengchi (GB20) , Yifeng (SJ17) and Wangu (GB12). Acupuncture was performed 5 times a week, 30 min per session for 8-week courses with a week's break between them. Results Both groups and their sub-groups demonstrated statistically and clinically significant improvements in swallowing function. Scores in the 2 treatment subgroups showed more improvement than in the control group. There was, however, no significant difference between the 2 treated subgroups in either their post-treatment or pre-treatment swallowing function scores. The cure rate in the treatment group was significantly higher than in the control group, but there was no significant difference in the total effectiveness rate between the two groups. Conclusion Swallowing training alone or combined with acupuncture can improve the swallowing of patients with chronic neurogenic dysphagia. The combination therapy was more effective. The improvement was not related with the course of the disease.

@#Objective To identify the risk factors for osteoporosis in patients with hemiplegia. Methods The total body bone mineral density (BMD) of 366 patients with hemiplegia in the stage of rehabilitation was measured with dual-energy X-ray absorptiometry. The data of the patients were analyzed with multiple logistic regressions. Results The incidence of osteoporosis was 15.3 % (56/366) in the cohort. ≥60 years old (OR=5.434, 95% CI:2.734~10.801), duration of disease (OR=1.046, 95% CI:1.013~1.080), body mass index (BMI) (OR=0.746,95% CI:0.622~0.840),ADL level (OR=0.618, 95% CI: 0.444~0.858) were independently correlated with osteoporosis in patients with hemiplegia. Conclusion Patients with hemiplegia in the stage of rehabilitation were at a high risk of osteoporosis. Aged over 60 years old, lower BMI, longer duration of disease and ADL level were independent risk factors for osteoporosis, which called for supervision and prophylaxis on osteoporosis.

@#Objective To investigate the changes of bone mineral density (BMD) and bone mass and the influencing factors in patients with hemiplegia.Methods The total BMD, bone mass, lean mass and fat mass of 366 patients (313 stroke cases and 53 TBI cases) with hemiplegia were tested by dual-energy X-ray absorptiometry, as well as the clinical features of all patients, were studied.Results The bone masses in the paretic side (upper limb, lower limb and trunk) decreased significantly compared with nonparetic side ( P<0.01); the differences of bone masses (%) in upper limb, lower limb and trunk between paretic and nonparetic side decreased with duration of disease or trauma ( P<0.01). The multivariable stepwise regression analyses showed that the total bone mass and total BMD were negatively correlated with age and duration of disease significantly (all P<0.01), and positively correlated with body mass index (BMI) significantly ( P<0.01), the total BMD was also positively correlated with lean mass ( P<0.01) and ADL level ( P<0.05).Conclusion The bone mass deceases in the paretic side significantly, and gets worse with the duration of disease. Age, duration of disease or trauma and BMI influence total BMD and bone mass; in addition, total BMD is correlated with total lean mass and ADL level.

ABSTRACT

@#ObjectiveTo investigate the effect of surrounding-acupuncture on scalp on hemiplegia.Methods90 patients were divided into three groups. First group were treated with surrounding-acupuncture on scalp, second group with traditional scalp-acupuncture, the third group was control group. ADL and Mannal Function Test (MFT) were used to evaluate the function pre- and post treatment. ResultsThere were no significant differences among the three groups on ADL and MFT at beginning, that were improved significantly after treatment. However, the first group improved more significantly than that of the other two groups for ADL and low limb MFT. The first group showed significantly improvement for Hamilton Rating Scale for Depression pre- and post treatment.ConclusionThe surrounding-acupuncture on scalp is effective method for hemiplegia.
